In consideration of the acceptance of this entry, I hereby, for myself, my heirs, executors, and administrators, waive and release Yavapai County, City Of Prescott, Antelope Hills Golf Course Prescott Area Celtic Society (PACS), its Games Committee, their agents, and representatives, successors, and assignees, for any and all injuries suffered by my participation in the The Prescott Area Celtic Society’s Summer Solstice of the Oak King Golf Scramble. In addition, I authorize PACS to take and use whatever photographs of me participating for whatever purpose they deem appropriate by an official PACS photographer.
